Integrated Case Management Services (ICMS) provides intensive and proactive outreach services for individuals discharged from psychiatric hospitals and hospitals with short-term inpatient units. These services help individuals access and utilize existing mental health and other community support services. Our community-based, trauma-informed approach facilitates a smooth transition and ensures a sustained connection with vital resources. The program operates 5 days a week with 24-hour on-call availability for Ocean County residents age 18 and older.

Integrated Case Management Services

The Integrated Case Management Services hospital liaison handles hospital and community referrals. Mandated hospital referrals are automatically enrolled, while community or other hospital referrals undergo eligibility review before scheduling client enrollment. The length of participation in ICMS is determined by the individual’s needs and the program’s determination regarding the ability to provide services.
Services are provided primarily in individuals’ homes, but also may be provided in community settings or PBHG offices when needed. The length of stay in the ICMS program is determined by the individual’s needs and the program’s ability to provide those services.Â
