Our program is specifically designed to provide case management and assessment services to individuals who are living independently but require practical support and guidance to maintain their well-being in the community.

Program Director: Donna Comerford, MSW, LCSW
Community Support Services offers case management and assessment services to individuals with a significant mental illness. The program is designed to provide practical support and guidance, with case managers working closely with individuals to identify their unique needs and develop personalized care plans to help them live as independently as possible in the community. Rehabilitative skill-building helps promote long-term self-sufficiency. With a focus on community outreach and collaboration, the Community Support Services program is committed to empowering individuals and linking them to available support in the community.
